You MUST have a qualifying condition under RCW 69.51A. Please bring any records documenting your condition. We are able to procure medical records for you for free from any previous clinic where you were seen. If you don’t have any records, please contact us.
$75 all evaluations* *that result in medical cannabis as a treatment option. Comes with 1 original authorization, and 2 copies. ($50 for evaluation + $25 for original authorization – also known as a medical marijuana card or green card)
